Converting a 4-string *neck* to a 5? It *could* be done, but the string spacing would be so close together that it would be damn near unplayable, to be honest.
Unless, of course, you *like* extremely narrow spacing...
You'd have to drill another hole and add another tuning key, first and foremost.
After that, you'd have to change the nut and start from scratch with a new one, and file it with 5 slots.
Once you put it on the bass, you'd have to adjust the pickups, etc., to accommodate the new string spacing, *plus* you'll have to tighten up the truss rod somewhat to compensate for the extra tension.
The bass would need a 5-string bridge and 5-string pickups, which may or may not be easily added.
The bottom line is that it would be cheaper and more effective to just get a 5-string neck to begin with, to be very honest.
